This service to acknowledges the journey our clients have made thus far and offers dignity and respect to shifting life circumstances. We offer navigation of support systems and insight into complex social dynamics. We understand your situation and concerns are unique. Now that you have found us, we are happy to support you in your next steps. It is our calling.

Why now?

Demographically, Colorado is aging in a way that it hasn’t before. As of 2019, 1 in 7 Coloradans is 65 or older. By 2050, the number of older adults in Colorado is expected to double to 1.7 million. Up to 70% of adults will need some type of support. According to the APA, “fewer than one fifth of older adults between ages 65 and 74 need assistance with activities of daily living, such as bathing or eating. This increases to 40 percent of men and 53 percent of women over 85 who need such assistance” link. Together we can help our family, neighbors, and friends continue to thrive in their communities.

Why is this a good idea for me/my family/my loved one?

We understand those with busy schedules want to be able to support their loved ones but often don’t know how. We are here to offer short-term care coordination and connection to resources to prevent costly (and maybe unnecessary) medical care, to support wellness, and to inspire meaningful connection with their community.

What can I expect:

We provide a comprehensive evaluation of the clients current and possible future social, emotional, and physical needs. Then we connect you with targeted community resources and specific programs to meet those identified needs. Our extensive resource bank is based on relationships, and we have historically partnered with concierge nursing, guardianship and conservatorship programs, long-term care management, Medicaid consultants, wealth management groups, and benefit programs for specific populations, including Nuclear workers and veterans.

Would you recommend this service to your loved ones?

Without hesitation, yes. As we watch neighbors, friends and family struggle to adapt to our increasingly complex lives, we want to protect their dignity and support everyone involved. It takes a village to support all ages of our community.

Can I just do a consult without the extended support plan?

Of course! You are welcome to meet with us for a 90 min consultation and at the end, we will provide you with a needs assessment and a care plan put together during that appointment. You are welcome to take those documents and do with them as you please. The cost for the consultation is $300 but it is applied to the extended plans if you choose to continue with our service.

I did a consultation and didn’t sign up. Things changed, and now I need help. What now?

As you’ve already completed the needs assessment and put a care plan together with our guide, you are welcome to sign up for our program within 90 days.

Why 90 days? What happens at that point?

We understand that care needs can change pretty quickly. Within 90 days, we can modify the care plan to reflect the current needs. After the 90-day mark, it is best practice to update the needs assessment and care plan to reflect what’s going on with the client to provide the best possible care and not rely on information that might have changed.
